Transfer Center at St. Luke’s

St. Luke's Transfer Center streamlines access to our providers and services with a single phone call. Our purpose is to connect providers for consults, admissions, transfers, and placement of patients to the appropriate care area.

Please have the following information prior to calling the Transfer Center:

  • Patient's date of birth, last and first name or St. Luke’s medical record number (MRN)
  • Prepare to fax face sheet to (208) 381-8882 to validate patient identity
  • Push films/studies so consulting provider(s) can review with referring provider

Please also be prepared to provide:

  • Your name, role, and direct call-back number
  • Referring provider name (spell last name, then provide first name)
  • Referring facility, department, and current room number
  • Diagnosis and requested specialty, if known (i.e., pediatrics, hospitalist, cardiology, etc.)
  • Preferred accepting facility, if known (Boise, Magic Valley, Meridian, and Nampa)

Special Instructions

Immediate Transport Requests

If immediate transport has been requested prior to connection with the on-call provider, the Transfer Center will bring Air St. Luke’s into the call to hear about the patient's: 

  • Current weight in kilograms
  • Isolation status
  • Intubation status
  • Current medication drips, if any
  • Cardiac monitoring, if any
Air St. Luke's will offer available resources or provide a timeline for updating the referring site.



Best Practices for Ensuring Successful Hand-Off

  • Send a copy of pertinent documents such as provider and nursing records, medication record, diagnostic images, slides, etc. with the patient
  • Call in a nursing report to the receiving unit prior to patient leaving facility (Transfer Center to provide)
  • If patient condition changes or re-connections with providers are needed, please call the Transfer Center back at (208) 381-2525
