St. Luke's in the Mountain Home Community

Our medical center has proudly supported the health of the Mountain Home community, Elmore County, and northern Owyhee County for 60 years. Our emergency department is open around the clock year round and our primary and specialty care providers offer you and your family compassionate care close to home. And, if you need additional specialty care beyond our local facilities, we’ll coordinate access to services and collaborate with your broader team of providers to keep you as healthy as possible.

  • Pediatric Cardiology

    Biography

    Melissa Agoudemos, MD, PhD specializes in pediatric cardiology with a broad range of interests including neonatal congenital heart disease, myocarditis, Marfan’s syndrome and other connective tissue disorders, and arrhythmias such as supraventricular tachycardia (SVT). She has additional interest in echocardiographic evaluation of congenital heart disease utilizing transthoracic echo and fetal echo.   

    Dr. Agoudemos received a bachelor's degree in biomedical engineering with a concentration in chemical engineering from Johns Hopkins University, where she graduated with honors and as a member of the Tau Beta Pi engineering honor society. She then entered the specialized Medical Scientist Training Program at the Medical College of Wisconsin to earn both her medical degree and a doctorate degree in physiology. During that time, she spent extensive time researching the process by which the body grows new blood vessels (angiogenesis). This led to multiple awards from the American Physiological Society.  

    Dr. Agoudemos completed her pediatrics residency at the University of Iowa, a referral center focused on treating the most critical pediatric patients across Iowa. Her residency included a specialized research track where she focused on the effects of maternal disease on the cardiovascular health of children. This was followed by a three-year pediatric cardiology fellowship at the University of Iowa. During this time, she continued her research and won the F. M. Kenny Memorial Award in Pediatric Research. Clinically, Dr. Agoudemos developed her interest in imaging with extra training in echocardiography.  

    Dr. Agoudemos is originally from the Midwest, and spent her early career working at Southern Illinois University. Now that she's in Idaho, she never plans to leave! She feels honored to provide specialized pediatric cardiac care to the children of Idaho.  

    In her free time, Dr. Agoudemos enjoys spending time with her two teenagers, three cats, and husband.

    Biography

    Nichole Aker, DO is particularly interested in all aspects of women’s health, including lactation medicine. She has also completed additional training in endoscopy and vasectomy. 

    Dr. Aker was born in Oregon but lived in China as an adolescent. Prior to her medical education, she graduated from Portland State University where she studied Mandarin. In her free time, she enjoys traveling, skiing, and singing. Dr. Aker is conversant in Mandarin and Spanish.

  • General Surgery

    Biography

    Robert Cavagnol, MD, MHA, FACS serves as president, St. Luke’s Clinic. His medical practice is currently focused on acute care surgery, and performs a breadth of surgical procedures including advanced laparoscopy, hernia repair, gallbladder removal, appendectomy, colon removal, and other urgent operations.

    Dr. Cavagnol is a Fellow of the American College of Surgeons. He has served as the president of the Southwest Missouri Chapter of the American College of Surgeons. Prior to relocating to Boise, Dr. Cavagnol served for 12 years as a general surgeon in Springfield, Missouri and earned the Young Physician of the Year award with Mercy Health System. 

    Dr. Cavagnol spent three years the United States Navy as a general surgeon at Camp Lejeune. During that time he deployed on the USNS Comfort, a hospital ship which supported New Orleans as the city’s trauma center following Hurricane Katrina. Dr. Cavagnol also deployed to Fallujah, Iraq in support of Operation Iraqi Freedom as a trauma surgeon.

    Biography

    Michael Charme, MD is a board certified OB/GYN working with all aspects of medical care for women from their teenage years through menopause. He has a special interest in providing personalized care for women throughout pregnancy. He also provides complete general gynecologic care, including surgery.

    In his free time, Dr. Charme enjoys photography, running, reading, the outdoors, and spending time with his family.

    Biography

    Karen Galvan, MD is a general surgeon whose practice includes laparoscopic procedures, breast surgery, upper and lower endoscopies, wound care, skin biopsies, and excisions. She welcomes questions from referring practitioners and is dedicated to providing excellent surgical care to her patients in partnership with their primary care providers. 

    Dr. Galvan is a member of the American College of Surgeons and the Society of American Gastrointestinal and Endoscopic Surgeons.

    Biography

    Cody Heiner, MD, MPH is the site medical director of St. Luke's Occupational Health and is board certified in occupational and environmental medicine by the American Board of Preventive Medicine. Dr. Heiner is also a certified medical review officer and provides comprehensive health-related services for employers and employees, including work injury medical care, drug and alcohol testing, employee physicals, impairment ratings, and immunizations.

    In addition to his medical education, Dr. Heiner earned his masters' degree in public health from the University of Iowa College of Public Health.

  • Occupational Health

    Biography

    Dave Landis, PA-C brings over 20 years of experience to St. Luke’s Occupational Health.  Before coming to St. Luke’s Occupational Health in 2013 he practiced in adult medicine, urgent care and dermatology.  Dave graduated from the University of Washington’s physician assistant program in 1995 and served in the Idaho National Guard for almost 10 years.

    Gordon Mack, MD cares for children with a wide range of congenital and acquired heart diseases. His particular interests lie in echocardiography, cardiac catheterization, and pediatric thrombosis. He completed his pediatric cardiology fellowship at Doernbecher Children’s Hospital and at the Hospital for Sick Children in Toronto. He trained in pediatric cardiac catheterization and pediatric clots in Edmonton.

    Jill Moughamian, PNP has clinical interests in plant-based nutrition and wellness and best practices for treating sick children.

    Jill earned her master's degree in nursing from the University of South Alabama and is certified by the Pediatric Nursing Certification Board. In her free time, she enjoys reading and the outdoors.

    Michelle M. Pehrson, FNP is a nurse practitioner at St. Luke's Clinic–Internal Medicine: Mountain Home. She enjoys working with patients of every age and background, and has a passion for providing compassionate and comprehensive care.

    Michelle was raised in a family with a medical background and always knew she would be in health care. She earned bachelor's degrees in microbiology from the University of Idaho and in nursing from Idaho State University. She earned her master's degree in nursing, family nurse practitioner from Idaho State University.

    Hala Rafla, PA-C joined St. Luke’s Occupational Health in 2011.  Prior to joining St. Luke’s she worked for the Department of Health and Welfare focusing on immunizations.  She studied medicine and surgery overseas where she worked in women’s health and urgent care. Hala holds a master’s degree in physician assistant studies from Idaho State University as well as a master in health sciences from Boise State University.

    Justin Smith, MD is a family physician with a special interest in obstetrics. He enjoys taking care of mothers throughout their pregnancies as well as continuing to see babies after delivery.

    Dr. Smith also practices every aspect of family medicine including wellness visits, care of chronic diseases, acute medical problems, long-term birth control via IUD and Nexplanon, and minor skin surgeries.

    Dr. Smith strives to provide research-driven, personalized, hometown care to the women of Mountain Home and the Mountain Home Air Force Base.

She completed her second master's degree at Idaho State University to become a nurse practitioner. In her free time, Anna enjoys the outdoors, exploring new places, and her family.
