Long-Term Care and Rehabilitation at St. Luke's Elmore

St. Luke's Elmore Long Term Care
Located in Mountain Home, our Medicare-certified, skilled nursing facility serves adults whose health concerns prevent them from living independently. We offer care during your rehabilitation after a hospital stay as you transition back to your home or assisted living. We also offer long-term residential living, palliative care, and hospice services for those with a terminal illness. We focus on maintaining a comfortable environment to help you or your loved one feel at home.

Our team includes nurses, social services, dietitians, and activity leaders who all work together to support you and your family’s unique needs and wishes.

We accept Medicare, Medicaid, and private pay. Please call us for rate information or to schedule a guided tour of the facility.

    We offer our patients respect for privacy and personal freedom, a personalized care plan that will evolve as your needs change, and personal relationships with our compassionate staff.

    We offer three home-cooked meals daily, as well as snacks throughout the day. Dietary preferences and special needs are monitored and adjusted by our registered dietitian. And, each resident’s eating habits are monitored to maintain optimum health. 

  • Daily Activities

    Patients may participate in arts and crafts, games, entertainment events, outdoor activities, and exercise sessions. We also offer access to our van, which is wheelchair-accessible and available by appointment for transportation.
