This class provides an overview of advance care planning, helping you understand key documents like Power of Attorney for Health Care, Living Will, and POST (Physician Orders for Scope of Treatment). Learn how these documents guide your family in making healthcare decisions on your behalf if you're unable to communicate. By aligning your values with your healthcare wishes, you’ll also be giving your family the gift of clarity, easing their decision-making during difficult times. This class offers three options to help you with advance care planning: 1. A virtual session on 4/9/25 from 12-1pm, providing essential information about advance care planning. 2. Two identical in-person workshops where you'll receive the same information as the virtual session along with hands-on assistance in filling out the necessary forms. - 4/16/25 from 1-3pm in Meridian - 4/23/25 from 1-3pm in Nampa Pre-registration required. If you register for the virtual session, a link to join the virtual session will be emailed 1–2 days before the class.
