Our multidisciplinary team consists of a neuromuscular neurologist, physical therapist, clinical nutritionist, speech therapist, and occupational therapist and is dedicated to patients with a suspected or established amyotrophic lateral sclerosis diagnosis.

This gives you a convenient, one-stop opportunity to confirm your diagnosis, optimize medications, and receive evaluations for therapy and prescriptions for medical equipment, as needed.

After your evaluation, the multidisciplinary team will create an individualized care plan and connect you and your caregivers with resources to help manage your disease.

What to Expect at Your Evaluation

Preparing for the evaluation: Your referring provider may give you an intake packet to complete before your evaluation at the ALS clinic. It is important to complete any forms that are given to you prior to your evaluation visit.

Scheduling the evaluation: Clinic staff will contact you to set up the evaluation, which will be during clinic hours on the third Friday of every month.

During the evaluation: At the evaluation, patients will meet with members of the multidisciplinary team individually. Each meeting will last approximately one hour per specialty, for a total of four hours.

After the evaluation: An after-visit summary and team recommendations will be added to your medical record and will be available to you and your referring provider via MyChart.
