Enjoy the peace and quiet of your own home, better sleep, and the freedom to welcome visitors on your terms—all while receiving the high-quality care you trust from St. Luke’s. Our Hospital at Home program brings a dedicated team of specially trained professionals to you, both in person and virtually.
We combine compassionate, hands-on care with 24/7 virtual support. From diagnostic testing and remote monitoring to ongoing treatment, our team ensures you receive comprehensive care without leaving your home. Upon admission to the program, we’ll set up a technology kit that securely transmits your vital signs and allows you to connect with your care team anytime via phone or video.
You’ll also receive in-person visits from a registered nurse—just like in a traditional hospital stay—along with two home visits from paramedics, nurse practitioners, or other healthcare professionals.
Your personalized care plan includes:
- Vital signs monitoring using a telehealth platform.
- Video visits by your care team.
- In-home visits by nurse practitioners and other health care professionals.
- Lab tests, mobile imaging, and IV therapies performed in your home as needed.
- In-home nutrition support.
- Collaboration with your primary care physician and specialists as needed.

Technology in Your Home
Computer Tablet
This is the device you’ll use for video calls, planning your day, and capturing photos. Your care team will use it for reviewing your vital signs and providing education.
WiFi Phone
This phone connects directly to your care team. Response time is typically within 40 seconds.
Personal Emergency Response System (PERS)
With this wearable system, you can make an emergency call to the command center from a wrist device.
Weight Scale, Blood-Pressure Monitor, and Pulse Oximeter
Just like in the hospital, these devices can be used to monitor the status of health markers.
Router
This device provides Wi-Fi and cellular networks in your home for connectivity to the command center. Or, you can use your home’s device and network.
Uninterruptible Power Supply
Provides battery backup in the event of a power outage.
