Intensive Lifestyle Medicine Program

A Healthier You Is Just Around the Corner

Lifestyle medicine empowers you with knowledge and skills to help you achieve better overall health and greater quality of life. Intensive lifestyle interventions have been shown to prevent, treat, manage, or even reverse many serious conditions, such as diabetes and prediabetes, obesity, high blood pressure, heart disease, mild cognitive impairment, osteoarthritis, autoimmune issues, depression, and more. This evidence-based, intensive program will help you make lasting changes that will help your body protect and heal itself.

Your team of experts will be led by Dr. Jennifer Shalz, who is board certified in lifestyle medicine and internal medicine. It will also include an advance practice provider, exercise physiologist, registered nurse, certified health coach, licensed clinical social worker, registered dietitian, licensed clinical psychologist, and nicotine dependence treatment specialist.

Getting Started

At the core of lifestyle medicine is the idea that there are six areas—or pillars—that form the foundation of your health: nutrition, physical activity, stress management, improved sleep, positive relationships, and avoiding toxic substances. You may choose to focus on all these areas or on a few. Our team will work with you to create a customized care plan that may include combination of online classes, group or individual visits, and in-person options. Depending on your level of participation, your program will last for 3-6 months. And because your success is important to us, support is still available for an additional six months after you complete your program.

    Health Care and Improvement Tracking

    Your care team will conduct regular medical check-ups, as well as lab draws, and progress surveys.

    You'll have access to a variety of specialists that offer health coaching on a wide range of topics, including nutrition, physical activity, and healthy sleep habits.

    We have a robust course curriculum to support you in learning new approaches to nutrition, cooking, stress resilience, emotional wellness, and behavior change.

    Our experts offer medically supervised in-person group exercise classes, as well as individual virtual exercise evaluation and plan development services.

    Give and Receive Ongoing Support

    You can be part of our "Sustain Your Change" support group after six months, where you can share tips, experiences, and encouragement with your peers.

Each journey through lifestyle medicine is unique. Some people may only need help in a few areas, while others may need a lot of support. These classes are open to anyone in the community hoping to make lifestyle changes. You may choose to take just one, a few, or a lot of these classes; whatever works best for you!
