Before your visit, review your new patient packet and complete all forms. Track your symptoms and write down any questions you have.
What to Bring:
- A list (or bottles) of your medications
- Your symptom notes and questions
- A support person, if possible—they can help take notes or ask questions
- Ask your doctor if you can record the visit for later review
