Enjoy your home’s quiet environment, better sleep, and visitors whenever you like while receiving the high-quality care you expect from St. Luke’s. Our Hospital at Home program brings a dedicated team of specially trained experts to you both in person and virtually. We combine compassionate, hands-on care with around-the-clock virtual care.
The Hospital at Home team performs diagnostic tests, remotely monitors your condition, and provides ongoing treatment. When you're admitted into the Hospital at Home program, our team will visit your home to set up the technology kit. The kit sends your vitals to your care team and makes it possible to communicate with them day and night by phone or video. You'll have in-person access to a registered nurse—similar to a traditional hospital stay—along with two in-home visits by paramedics, nurse practitioners, and/or other health care professionals.
Your personalized care plan includes 24 hours-a-day, seven days-a-week access to your care team, either in person or via video and telephone, as well as:

This phone connects directly to your care team. Response time is typically within 40 seconds.
