Community Health Needs Assessments

The St. Luke’s Health System Community Health Needs Assessments, or CHNAs, help us better understand the most significant health challenges facing the people and families in communities across our region.

The comprehensive analysis allows us to pinpoint, assess, and address our communities’ most critical needs. St. Luke’s uses the information to guide the types of health improvement programs we provide and support with community partners. The goal is to address the top health needs, provide the best care, improve overall health and wellbeing, and lower the overall cost of health care. In addition to extensive national, state, and local health data, the CHNAs incorporate feedback from interviews with community representatives who help us identify and prioritize our health needs. The community representatives also suggest initiatives to help us address the health needs. 

St. Luke’s analyzed more than 60 potential health needs and health factors. Researchers looked at health factors across four categories: health behaviors, clinical care, social and economic factors and the physical environment. The potential health needs within each category are prioritized, and the needs that rank at the top of each category are considered to be our most significant health needs.

St. Luke’s collaborates with the people, leaders, and organizations in the communities we serve to carry out an implementation plan designed to address the most pressing community health needs identified in this assessment. Utilizing effective, evidence-based initiatives, we work together with trusted partners to improve community health outcomes and work toward the goal of attaining the highest level of health for all people, where everyone has a fair and just opportunity to attain their optimal health.
